Applications for crew documentation or certification for service aboard Zanzibar-flagged MODUs and offshore units must include the documentation specified below. To ensure efficient processing, all submissions must be complete, accurate, and compliant with applicable maritime regulations and international standards, including relevant STCW requirements, where applicable.
Required Application Documentation1) Seafarer Application & Medical Fitness• Completed seafarer application forms
• Valid medical fitness certificates not older than two (2) years
• Valid drug and alcohol test results not older than one (1) year
• Two (2) recent colour passport-style photographs
Photographs must be clear and suitable for official documentation purposes.
3) Officers’ Licences & Qualification EvidenceOfficers must provide:
• Copies of valid Certificates of Competency or professional licences issued by a recognised authority
• Documentary assessments or competency evaluations, where applicable
• Relevant training and course certificates appropriate to the rank applied for
All certificates and licences must be valid and unexpired.
4) Ratings & STCW ComplianceCrew members serving in rating capacities (including but not limited to Able Seafarers, Motormen, Oilers, and similar ranks) must submit:
• Copies of training certificates consistent with STCW standards for the respective rank
• Documentary evidence of qualifications relevant to assigned duties
Seafarers applying for certification or endorsement related to survival craft or lifeboat responsibilities must provide:
• Documentary evidence of service clearly specifying the rank held and duties performed
5) Employer / Owner Reference Letter• Reference letter issued by the vessel owner, operator, or authorised manning agency
Such letters should confirm the applicant’s association with the vessel or intended service.
6) Cover LetterA formal cover letter must accompany the submission, specifying:
• Name of the vessel or offshore unit
• Names of the seafarers included in the application
• Submitting entity or representative
• Delivery instructions, where applicable
Upon completion of the application process, authorised signatory or signatories must review, sign, and date the required documentation in the designated sections.
Electronic submission may facilitate expedited preliminary processing; however, applicants may be required to provide original signed documents where mandated by Registry procedures.
Compliance & VerificationAll applications are subject to identity verification and document authentication protocols.
Incomplete, inconsistent, or unverifiable submissions will not be processed.
Review and processing timelines commence upon receipt of a complete and compliant application package.
